Thank you for your interest in Bluegrass Academy Child Care.  We realize choosing a child care center is one of the most important decisions you make but can also be the most stressful.  In order to simplify the enrollment process we have provided all of the necessary forms below.  The only exception is the Child Medical Statement, complete it only if your child has specific dietary need due to allergies or religious beliefs.  The form is to be completed by your physician or religious leader.
